This is the State of Illinois sponsored event for the Pfizer-BioNTech COVID Vaccine for children 5-11 years old.

The library cannot take registrations.

If your child needs both doses, please register for both dates individually.

A consent form will need to be signed before the vaccination can be administered. You can fill it out the day of, or in advance here.

If you can only come to one of the dates, a clinic volunteer will direct you where to go for the second shot.

Registration is highly encouraged, though walk-ins are welcome.

IMPORTANT  - PLEASE READ:  Appointment Guidance:

1. Bring your ID, insurance card (if available), and CDC card (if you have one).

2. Please do not come if you are not feeling well. Call 800/325-1812 to reschedule.

3. Minors under 18 must be accompanied by a parent or legal guardian.

4. For patients with disabilities, only one person will be allowed to escort you. Please notify the clinic volunteer checking you in and come to the front of the intake line. Patients with disabilities will be given priority.

5. Do not skip regular doses of medications.

6. Please make sure you are not fasting.

7. Please write down any side effects you may have experienced with previous COVID vaccinations and report it to the clinic volunteer. Specifically, take note of any allergic reactions, shortness of breath, prolonged fever, or body aches.
